Nominate a beneficiary

Nominating a beneficiary can help make sure your benefit, including your account balance and any insurance cover you’re entitled to goes where you want it to in the event that you were to pass away. You can make a non binding, or a binding nomination, and you can only nominate certain people as your dependants. Find out more about the rules in our Nominating Beneficiaries factsheet.

Complete all fields.

Witness

To make your nomination binding, two people who are over the age of 18, who are not beneficiaries, must witness you sign your declaration. They must sign and date the form at the same time as you.

Submit

Send your completed form to: Mine Super Locked Bag 2020 Newcastle NSW 2300.

We'll update your account

We’ll update your account within five business days of receiving your request. If there are any problems with your nomination, we’ll get in touch to let you know what you need to do to make a valid nomination.

Things to consider

Your nomination will continue to apply unless you cancel or change it (or, if you make a binding nomination, it expires), so it’s important to update it if your circumstances change, such as separating from your partner, marrying or having children.
